Does This Sound Like a Bad Motor Mount?

Okay so i've known for a while something was wrong, here are the symptoms. I thought it was the trans but now i think its the mount.

-Motor jerks forward than back when shifting from park-to-reverse or park-to-drive
-'hiccups' when shifting from 2nd to 3rd, and a miniscule amount from 3rd to fourth
-Doesnt do it when cars on incline, or going from reverse to drive, or if the wheel is turned to the right all the way
-Car is now stalling when at a complete stop. Car drives fine in everything else, but when completely stopped for a minute the car will shake, the electric will begin fading and if i wait to long the car will shut off.
- Does not do ANY of this on a cold start, only when engine has been on for about 15-20 minutes.

Anybody familiar with a problem like this? Got the codes red most of it were O2 sensors which i do not think would cause these things to happen.

07 cobalt 2.2 auto. 110k miles.

Sensor voltage fuse, dirty throttle body and rear motor mount.
