Most likely a connector contact issue. Re-seat connectors if this solves thr problem add dielectric grease to connectors in question.

Only one on the TCM. The 2 blue/gray on the BCM are usually the culprits. Also sometimes the wiring harness will rub and wear through close to the thermostat housing. Check if plastic armor is intact.

Does your code often reappear therefore trigger the CEL on 2nd appearance or is it infrequent? How does the car actually behave? There is one thread that explains it: 68121-my-car-jerks-hard-when-shifting.html and the one I posted: 101948-hard-shifting-no-iss-start-no-codes.html.
